Who is Eligible?

 

A person is eligible to recieve a Viva Pack if they:

live in a community that has a Viva Pack service*

have been diagnosed with cancer and are either currently receiving treatment or require support/care as a direct result of the cancer or treatment

are not an inpatient of a hospital

are not staying in an accommodation service unless it is supported accommodation operated by Can Assist (i.e. Jean Colvin Cancer Centre or Lilier Lodge)

do not meet the eligibility criteria of existing community support services or existing services cannot offer a timely or adequate level of service to meet their needs

have no, or limited informal supports

are affected by carer stress

have complex care needs (i.e., care needs that can only be met by a co-coordinated package of care services).
