Request a Viva Pack Service

Anyone can request a service for themselves or on behalf of a person affected by cancer.  Requests for service can be made by community or health care professionals, friends, relatives or Can Assist Branch members. The local community options program will also be a key referrer to this service.

There are three ways to make a referral

1. Call and speak to the Viva Care Coordinator – 1300 226 277 (cost of a local call from a landline)
2. Email request to vivapacks@canassist.com.au
3. Complete the Initial Enquiry Form
